Referral Process
Referrers for either Supported Living or Domiciliary Care need to complete a referral form and attach a risk assessment. This should give consideration to the individual’s willingness and suitably for shared supported living from the perspectives of those affected i.e. referrers, clients, other tenants, community and landlords. It should also include what crisis contingencies/support will be in place to mitigate and manage presenting difficulties.
How Our Assessments Work
Assessments for Supported Living or Domiciliary Care involve a discussion with the individual about what support needs they have under the Care Quality Commission’s ‘Essential Standards of Quality and Safety’. An agreement will take place about how support can be delivered constructively in order to safely and appropriately achieve a positive outcome. Following assessment, referrers are provided with a proposed support plan.
